This Cancellation Form must be submitted 30 days in advance of the billing cycle (1st of each month). You may still be responsible for payment of tuition for one additional billing cycle depending on when this cancellation notice is submitted. We do not pro-rate cancellations, if you submit this form mid-month you will be responsible for one additional month of tuition. Should you wish to re-enroll, you may do so anytime through the Customer Portal (Jackrabbit).
Please indicate the starting date you wish the transfer to take place. Provided there is space in the desired class, transfers may be processed immediately unless otherwise specified below. If the desired class if full student will be waitlisted for the new desired class and remain in their current class until an opening becomes available.
